Kayak/Canoe Trip Waiver and Release of Liability
Yay! If you're reading this, it's because you've signed up to participate in the Kayak/Canoe trip on June 9th (If you or someone you know hasn't registered you yet, you've taken a step to far! Go back to the Event Registration page or Checkout).
Now, You're one step away from enjoying the lake with us. This is the legal stuff and we have to do it so bare with us. Please submit one response for each participant of the event. If you are unsure if your response went through, please try again or contact Lily Bonwich. If you would prefer to sign a paper form on the day of the event, please let us know... because otherwise we'll keep sending you a link to this page! Thank you for complying with our insurance requirements... we're almost to the fun part!
I recognize that because of the inherent hazards of this activity, that I may sustain some injury. Such risks include, but are not limited to, drowning, capsizing, head injury, injury from exposure to the elements, animal bite or laceration caused by animal (including bird, mammal, reptile, fish and amphibian) teeth, beak, claws or talons, infection from insect, spider or tick bite, snake bite, irritation or allergic reaction to poison ivy or other noxious plan, dehydration, sunburn, heat exhaustion, sun stroke, hunger and starvation. On behalf of myself and my child or ward, I assume all risks associated with this activity (including known and unknown risks not mentioned above). In the event that I am injured and my next of kin cannot be contacted, I give my permission to the attending physician to render such treatment as would be normal, and agree to pay the usual charge for such treatment. If I am signing as the parent/legal guardian of a participant, I recognize that because of the inherent hazards of this activity, my child may sustain some injury. In the event that my child is injured and my spouse or I cannot be contacted, I give my permission to the attending physician to render such treatment as would be normal, and agree to pay the usual charge for such treatment.
I now release the Friends of Lake Monroe, and its employees, agents and assigns; the State of Indiana Department of Natural Resources; David Rupp and IndiGoBirding; Dan Clark; and the event organizers and sponsors for any and all claims for personal injury and/or property damage that may arise from, or be in any way connected to, my or my child’s participation in this activity.
I understand that this release applies to both present and future injuries, and that it binds myself, my spouse, my child, and the heirs, executors and administrators of each of these persons. I have read this release and understand all of its terms. I sign it voluntarily and with full knowledge of its significance.
